Be sure only Surname,Given Names is checked on the top part
Click Customize, Make Birth Date & Death Date the first 2 fields
Put check marx next to them
Click Preview

That sort of worked, I can work with this. I was actually looking to create a list, one name under another with their YOB/YOD next to them. But this will do.
