I don't get any names in parentheses on the Relationship Chart - I'm using the latest version 5.0.0.170. I don't recall names in parentheses at any time. I have a saved chart from the last version 5.0.0.161.
Try deleting RelChart.usr in the Legacy program folder and choosing your options for this report again.
On the screen where you choose the options for the Chart you can change the title.
