There are many blank family group sheets out there to print.  You can go to
http://www.cyndislist.com/supplies.htm to find links to different styles and
choose the one you that best suits you.

Getting ready for this year's reunion and will need blank family sheets. I
have asked before to no avail as I need a space for e-mail, phone and
address.

Maybe some other blank report gives these or how to adapt existing one?

Could do in Access, Word or Excel, but have to type in.

Anyone have such a sheet they have designed?
