Regarding your first question, I have been doing the following:

1. Scan the document in question
2. Create a Source (or Source Detail) for the document and attach the
document to the Source (not the Event).
3. I type the complete (or significant) data from the document into
the Source Text (with a blank line at top so it reads better in the
report)
4. I (optionally) highlight the pertinent data on the Source Text
(Name, Date, etc.)
5. Then, each time I reference that document, I use the identical
source using the source clipboard (it must be IDENTICAL) this causes
each entry to use the same footnote and thereby prevents the source
from printing multiple times.
6. I make sure that the Source Picture and the Source Text are checked
(enabled) for printing.

Using this method, when I print the Family Group Sheet, each item on a
given certificate has the same footnote number and all sources appear
at the end of the Family Group Sheet.  The entry for the foot note
will contain the source name & details, the Source Notes and a small
picture of the document.  So even though the document is very small, I
can easily read the (pertinent) detail from the document in the Source
Notes.  If I ever question what I have typed into the Source Notes, I
can review the actual document to verify its correctness.  I find
putting the document picture & Text in sources rather than in Events
makes the report easier to read.  Also, if desired, I can easily
eliminate the Source Picture, Source Notes, or even all sources in
future printings.

I do not publish to the web, so I don't know if this would work for
that purpose.
