No.  This happens to me once in a while, too.  The only remedy I know is to 
run the vr3-use-default-layout command.  Once the position is back to 
normal, I haven't been able to make it misbehave on purpose.  Very 
annoying, because it's nearly impossible to debug.

The only clue I know is that in this condition, the title bar of the free 
floating window does not have the name of the outline.  If you open a free 
floating window using the splitter context menu, it does have the outline's 
name.
