To use Apple's X11 for building Lesstif, he'll also need to install the X11SDK package (it's somewhere in the X Code folder or a subfolder of that folder on the OS X install disk (sorry that I can't recall the exact location)). That SDK isn't installed by default when one installs X Code or the user X11 package.
