#2189: MC-4.6.1 UTF-8 patch is now good enough

Finally, Debian managed to knock a large percentage of bugs out of the
UTF-8 patch for MC. The new instructions are available at the MC Wiki
page: http://wiki.linuxfromscratch.org/blfs/wiki/MC
The patched MC works acceptably well (but not perfectly) in both UTF-8 and
traditional 8-bit locales. However, it is better to play safe and include
a new page about the patched MC, instead of adding the patch and changed
instructions on the existing page. So that there are pages in the book
with the titles "MC-4.6.1" and "MC-4.6.1 with Debian UTF-8 patches".
