Do you have an older kernel, and is it *still* ok with that ? I suspect you might have a problem with the disk. Do you have smartmontools installed ? Or, possibly, libatasmart (I found that one hard to use when I tested it). I hope that suspicion is wrong, because recovering from disk failures can be painful.
