I think is the index variable that causes this behaviour.
Try something like this
%4C %Z %{!%b %d} %-15.15n %?M?(#%03M)&(%4l)? %?y?(%.20Y) ?%s
I don't remmeber which part is exactly right now,if you want I will
try to revised my docs.
But you can try inside of mutt different settings and you will
eventually find the difference.
