OK to push the last version? Also fixes a bug in "manual poc refresh"
detection/handling. It can now basically handle most of the tandberg
samples even without the mmco reset bitstream elements.

Looks way better, I'm a bit concerned regarding frame flushing but I'm afraid there isn't much to be done.
