The Evil Plan is hereby declared (mostly) completed. All the remaining decoders are now converted and seem to work well enough.
This set contains: - a resend of the core lavc changes, now with buffer pools. - a resend of the pthread patch, now with dynamically allocate array for released buffers. That is needed because there is now much more reffing and unreffing going on, so the current static array can easily overflow. - conversion of mpegvideo and h264. Please review.
