Easiest thing to do is to work your way through the example in ffmpeg/libavcodec/apiexample.c You should be able to build and run it, and then modify it as required. Without looking at your code, it is hard to work out your bug, but at a guess you should probably pay attention to how you are packing your AVFrames before encoding.
