Not sure these will satisfy all of your needs, but both of these
companies are run by people who care about privacy and security.

Greenhost (Netherlands) has a great reputation and a smart group of
people working there, who are often at hacker/security/internet-freedom
type events:

RimuHosting (New Zealand): I have been hosting here for years, and they
are extremely reliable... not sure where NZ stands on your data terms
though.

FlokiNet (Iceland) is a newer company, but they say all the right things
on their site, and are planning to setup an OStel secure voip instance
for us, which makes me think they know what they are doing:
