This is not covered by HUGETLB_DEBUG. Adam and I discussed this a few days ago, and we're thinking about adding a diagnostic module (diagnostic.c or something), which will try and diagnose things like this (we have a few other common failure scenarios that we've run into so far, iirc), when applications fail (and HUGETLB_DEBUG is set). Not there yet, but on the horizon.
