I get O(h^4)/O(h^3) L2/H1 convergence in ex14 after changing that to HERMITE/3... although I also had to bump down the linear solver tolerance before it would be sufficient to do enough uniform refinements to make the accuracy clear. Are you sure your solver tolerance is sufficient? That's a more likely problem than quadrature error.
