HERMITE/3...

accuracy clear.

Are you sure your solver tolerance is sufficient?  That's a more
likely problem than quadrature error.  For getting quadrature accuracy
we've got "FEType::default_quadrature_rule()" which is sufficient 99%
of the time, but for algebraic solver accuracy we've got "ha ha,
you're on your own" which has failed for me on multiple occasions.
