There isn't... but there is a MOOSE test you can run
(moose_test/tests/preconditioners/pbp/pbp_adapt_test.i).  This is all with
PETSc 3.3.0-p4 BTW.

If you really want a raw libMesh example I can probably make one.  Let me
know.

Well... looking back through the log, John did change something in there
back in December (around when this started failing), but we didn't notice
the problem until switching to PETSc 3.3.  I can only conclude that we were
somehow "getting away with it" all that time.

The fix with Destroy and Create works fine (even with Hypre).... and I'm
going to change stuff on my end to make sure it doesn't happen every
nonlinear iteration (I just looked at the code and we are calling init()
way too much currently).
