Vikram just managed to trigger a nasty libMesh bug that I think could
manifest in more subtle ways for anybody doing DirichletBoundary
objects on a refined mesh.  I'll probably be backporting the fix (and
an AdjointRefinementErrorEstimator fix or two while I'm at it) to
0.9.2 on Monday.

You'd think we could have just one release that didn't quickly require
us to switch to a major.minor.micro.embarrassment_level version
numbering scheme.

Let me know if there are any other fixes worth patching on to the
release while I'm at it.
