I've used a couple of different tools... but around here we've settled on Totalview. It is expensive, and can be a pain.... but it is powerful enough to allow you to get your work done. That said, I am interested in Eclipse+Extensions but haven't tried it.
In my personal day to day work I actually just use XCode on OSX to do most of my debugging... only firing up Totalview when I _must_ debug something in parallel.
