Is there any quick way you could summarize the error for us?  In the
past we've found that knowing about common classes of subtle user
errors (I'm thinking of "user adds AMR to non-adaptive code, forgets
to add constrain_* call to assembly routine" but there have been
others) is useful for helping future users with the same error and for
designing future APIs to render the error moot.
