Bi/triquadratic LAGRANGE dofs are higher-order (for loose values of
"high" with edge-dofs which shouldn't need extra_hanging_dofs() ==
true.  It looks like BERNSTEIN and SZABAB may be bugs, though.  Would
you try changing your fe_interface.C and see if that fixes the
problem you've encountered?
