The user OS is Win 7 and Windows XP with all patches installed. LibreOffice is used in the last stable version. We tried it on severel systems to find out side effects, but we got the same results. If you open the PDF document with Adobe Acrobat Reader, you can fill out the formular. But the text is printed and shown as you can see it in the example. It doesn't matter, if you make the control higher (one line only), the text will appear at the bottumn. I think that you add some additional paragraph setting during the export to PDF and so the text doesn't fit into the fields.
