test files to reproduce the bug
under some conditions, checkbox - form fields are in wrong positions or not at
all in an exported or printed pdf document.
Way to reproduce:
Create a new empty calc document.
Choose forms design mode, create checkboxes, modify them to have no label, exit
forms design mode, check some of them.
Export to pdf or print using windows' pdf printer.
(formfields-test1.*)
Result: only one checkbox is in the pdf, and shifted from its correct position.
Modify the spreadsheet (formfields-test2.*)
Export to pdf or print using windows' pdf printer.
Result: no checkbox is in the pdf.
