I spent lot of time trying to create different alphabet indexes - oops.
User-defined indexes are like contens, page ordered - this have absolutely no
sense.
While large book or report creation, typically task is creation several
alphabetical indexes, for example:
- peoples personal names,
- geographical names,
- names of companies, vessels, brands and so on,
- terms,
...
All they should be 123...ABC... ordered to find what the reader need, not
page-оrdered.  Only content should be page-ordered.
