For spreadsheet output the printed version includes everything correctly. When
you export the same spreadsheet to a pdf none of the heading lines (from
Format/Page/Headers) are included, just the spreadsheet body including the
column headings appear. This is a long running bug going back many versions
that I thought would have been found and fixed by now.
