In the process of debugging I found that the numbers to the left of the outline text overlap the 0.5"/1.27cm text-indent/margin-left value in the style for the third level. This is not the case for levels 1 and 2. And indeed, when I use the mouse in Word to adjust the indentation slightly to the right so it is past the numbering, the text returns to its expected position immediately next to the numbers.
