After the crash, I went to the URL that was specified for the crash report, but no report was there—"Page not found." I want to provide you with additional information, but I am not sure what I can provide. Just to confuse the issue, I just created a test document and then issued the commands to create a ToC. This time it worked. Wish I understood. I have now attempted to insert the ToC into my working document (cannot submit, private), I again obtained a crash. All chapter headings are set to Heading 1, I do not understand the difference. Please give me a clue as to what I might be doing wrong.
