Thanks for getting back to me so quickly, I'm impressed.  I understand the
image/frame situation, it is that relationship that is changing.  I did as you
asked but also could not reproduce on that document.  But the clip that I
attached to the bug report shows the behavior.  It doesn't happen all the time,
but enough that I am replacing images with every open.  When I last opened the
large document none of the images were offset, but many were running into the
margins even though all had been set and saved to stay within the text area. 
And in this case it often does not offer a Properties popup in which to
re-enable that setting.  It's odd, the properties and how they are presented
(what is available on the sidebar, whether or not a popup is available) seems
to change depending on the image.
