I found a workaround allowing to create TOC containing [apparently] only levels
m to n. Max level n is already configurable in the TOC insertion dialog.

Eliminating levels 1 to m-1 is only visual. The trick is to completely remove
all descriptors in the structure line for 1 to m-1 and to associate these
levels to a specific paragraph style instead of Content 1 to m-1. This specific
paragraph style has Hidden attribute.

The first part of the trick is not really necessary but why would you generate
something to get rid of it immediately. Since paragraph contents is hidden, the
paragraph is ignored when laying out text and "missing" levels don't occupy
space.

Tricky but works. Perhaps not stable and may not work in future releases
because it relies on an undocumented behaviour (removal of hidden empty
paragraph, even its above and below spacing).
