E.g., in Colibre icon set (default on Windows), and Elementary icon set (default with Gnome), Breeze, and Yaru, it is a red dot. When the file is unchanged (e.g., when you just saved it), the decoration disappears. This is exactly what *is* informing us whether the file was saved or not. So you need to provide information, what you see, what icon set you are using, etc. It could be an issue with a specific icon set; or with some mode (e.g., dark or high contrast); or some rendering machinery.
