Description:
When creating a custom heading for a document, applying it to selected text
does not always show it as such in the navigation pane. This tends to happen
when reopening a document with this custom heading, as opposed to creating the
heading in the same session it's used.

Steps to Reproduce:
1. Create a heading based on a preset one (e.g. Heading 1)
2. Apply it to selected text
3. Close document
4. Reopen document
5. Apply custom heading to another set of selected text

Actual Results:
The first custom heading shows up in the navigation pane under "Headings," as
expected, but the second heading after reopening the document does not (its
frame may still be under "frames" though).

Expected Results:
Both selections with the heading settings applied should show up in the
navigation pane under the "Headings" list.
