If I have a read-only file - I can't change its contents, but - I can delete it from the directory containing it. It is the _directory_ that needs to be read-only to prevent deletion of files. Similarly, a Table-of-Contents or Index, protected against manual changes, should be deletable altogether from the document containing it, and only if the document is read-only should that be prevented.
