Now, you might ask: "But what 'headings' do you mean, exactly? And what will we
'hang' these entries onto?"

Well, it needs to be whatever can make it into the Table of Contents. Which, in
fact, means all paragraphs in the document, since one can have any paragraph
style picked up for the ToC. So, actually, each paragraph should be able to
have non-displayed content that serves as its ToC entry.
