If you're really using object "6 0 R" as the reference in both those arrays, it's definitely incorrect. The reference in the "D" array must be to the page, the reference in the "SD" array must be to the StructElem object you're linking to. But only the first item in each array needs to be different, the others (the coords on the page) I would expect to be the same.
