I tried to report the bug 2-3 years ago, but I did not found the exact cause that is generating the duplicated conditions. What I suspected at the time was copy - paste of the formatting from the rows with conditions on the new rows that are not formatted as conditional by default. You mentioned that if you insert A6, a new row automatically you get the conditional added to the new row, but it is not always like that. If you right click in the left on A5 and add a new row from the last row you get the conditional extended, BUT if you just write something on an existing row (like A6) you will NOT get the formula extended to a new row. So, in this case I would copy the format from A5 to A6. And sometimes the formula duplicated.
