Description:
when creating an index entry (for the same type) multiple times it should
result in only ONE final entry. this is esp. problematic when using "apply to
all other occurences". when adding new text and reindexing it, the oldest text
parts stack up with a long list of the same entries.

Steps to Reproduce:
1. in a doc, select a word, "insert index entry".
2. press insert multiple times, close dialog
3. rmb over that indexed word, "edit index entry"

Actual Results:
a long list of the same word

Expected Results:
it should have been entered (inserted) only once
