It's definitely something about your setup that is causing the problem. I just
purged libreoffice 4.0.3 and 4.0.4 completely - installed 4.0.4 and did a test
case that worked fine.

Some hints for you to diagnose on your environment:

1. Try on a live boot of Ubuntu - this guarantees default everything

2. reset your profile: https://wiki.documentfoundation.org/UserProfile

3. If you installed libreoffice in any other way other than the official
repositories and/or ppa please purge it completely and stick with the official
repos/ppa's (ie. don't download from libreoffice.org)

4. Try 4.1.0 which is currently out
