I have the same effekt under Windows (LibreOffice 4.2.0.4) and under openSuse
13.1 (LibreOffice 4.1.4.2 (410m0 Build2)).

Steps to reproduce:
1. Create Document with one paragraph as "Outline 1", 
2. add Table of contents. 
3. add index
4. Set index mark onto the word in the Outline also selecting an option "Apply
to all similar texts".

As a result two index marks are added. One of them - on the TOC entry and
another one on the outline. 
The entry in the TOC cannot be removed.
Expected behavior: 
1. (better one) No index mark should be set on the indexes/TOCs etc.
2. (the valuable one) the entry in the TOC should be removable.

There is also another effect (at least in the 4.1.4.2) - when navigating
between marks, the enabled-state of the OK and Remove buttons goes brocken (see
example document attachached)
