Problem description: 
I have a writer file with 2 pictures in the header. If I save this file as docx
and open these with Word 2010 then Word tells me that this file cannot be
opened because of a xml error or because the file is corrupted.

I tested this with a new writer file. I could not reproduce corruption, but I
could reproduce missing/unreadable pictures in the word documents. Writer
behavior seems not to be deterministic, since sometimes the same step produces
a different result. But repeated editing always leads to missing/unreadable
pictures in the word document after a few steps.

The enclosed zip file contains test2.odt and test2.docx which does show all the
pictures. I did copy/paste one picture from the default page header into the
title page header and saved as test2-1.odt. Save as test2-1.docx, which does
not show the picture on the default page anymore.

Steps to reproduce:
1. Create new writer document with a title page and a default page, both with
headers enabled
2. Insert a picture in the header of the default page, save a copy in docx
format
3. Insert a second picture in the header of the default page, save another copy
in docx format
4. Insert a picture in the header of the title page, save a copy in docx format
5. Repeat step 2, 3 or 4 in various manners

Current behavior:
After step 4, at least after a one of few repeated steps, the docx document
does show placeholders/frames of (some of) the pictures stating "This image
cannot currently be displayed". The size of the docx file is smaller then than
the size of the docx file before the last step, although a picture has been
added.


Expected behavior:
The docx files always show the pictures.
