Problem description:
1. The page numbering is broken: cf. screenshot #1
2. the "Evaluate up to level" feature cannot go beyond level 5, despite
configured otherwise: cf. screenshot #2
3. The "Edit Index/Table" brings up a "Insert Index/Table" windows: cf.
screenshot #2
Regarding issue 1., for the first time in the document, a "Heading7" title is
inserted in the area where the page numbering restarts: this fact might trigger
this issue.
