Problem description:
When numbering headings, from level 2 headings only show the number of their
own level - with the exception of level 3 showing nothing at all.
Steps to reproduce:
1. enter text and turn to headings using ctrl+1, ctrl+2, ctrl+3 on various
paragraphs
2. number headings using format, bullets and numbering, then the third tab and
the third item in the second row.
Current behavior:
wrong/missing numbering (even in the preview in the dialog)

all headings get numbering scheme
