I too am having a similar issue as this (as of Version 4.3.0.4).

Where if I copy/paste data from within the sheet (seems to cause the issue
every time), edit a formula or change data in a cell (seems to cause it
frequently, but not always), or even at times just arrowing across cells (rare
chance of causing the issue), I even once fixed the range on the conditions,
save the file, closed libreoffice Calc, and open the file back up and the issue
occured again.

The range which has a conditional formatting active across those cells seem to
become discontinuous
For example the Conditional Formatting Range is set for: J11:J106, I make an
edit to a cell within that range, and I'll go back to Manage Conditional
Formatting and I'll see a number of new separate rules for:
J11, J45, and J101
And the original rule now has: J12:J44,J46:J100,J102:J106
(And this is in the best case scenario where range is semi-contiguous) 

And issue gets more confounding once it goes across multiple columns (IE
AB8:AG100) Where incomplete lines of rules are made and become non-contiguous
and cells are skipped and become unformatted.
