Importing the document found on the following website:
http://www.sarafi.com.au/images/sarafi_files/money_transfer_form.docx
This document's layout is broken. There should be only a single page and also
the form widgets are out of place/incorrect, I am guessing the DOCX file was
created with MS Word.
If deemed helpful, I could attach a PDF printed from MS Word as a reference.
