The Insert > Media submenu was created in bug 53682 but additional improvements
should be made to it, now that it has been created.
1) Insert > Media > Image should be moved to Insert > Image, as inserting an
image is the 2nd most used item selected in the insert menu, so it shouldnt be
found in a submenu.
2) Insert > Audio/Video was moved into the Insert > Media submenu, so it can be
split apart into Insert Audio and Insert Video.
3) Image related items should be placed at the top and audio/video items placed
at the bottom.
