This is on Windows 8.1 and LibreOffice Writer 4.2.3.3. I haven't noticed a definite pattern to when it happens, but the original poster's description seemed to ring true for me—it works fine to start with, but then, after doing a copy/paste within Writer, it will no longer paste from an external source (e.g. an e-mail).
