Right click TOC - Update index/table fixes the problem on 3.5 and 4.5. Save and
reopen retains the TOC numbers just fine.
I'm setting this to INVALID.
Franco: I hope you get the same result. If not, you can reopen this and set to
UNCONFIRMED.
