This is an annoying bug. Which seems to be associated with HTML documents.

I created heading 3 style with numbering. I set Tools, Outline Numbering to
1,2,3 with after )<space>. 

Everything works fine until I close the file. When I reopen, already numbered
Heading 3 styles are fine, but if I create a new line the number is missing.

1) First heading
2) Second heading
Third heading created after file close.

If I go to Outline Numbering for Heading 3 it is reset to default (no
numbering). If I reconfigure it I now get

1) 1) First heading
2) 2) Second heading
3) Third heading created after file close.

and I have to manually remove the extra 1), 2), etc..

Thereafter, everything works fine until I close and reopen the file. I have
tried to duplicate with with a "clean file" but the Outline Numbering menu item
is no longer available.
