document containing 2 tables, each with two rows with merged header cells
When Tables are saved as HTML from Writer, with headers on multiple rows (Table
Heading style) which also have merged cells spanning across rows, merged header
cells aren't included.
This is because <thead> doesn't include any more than the first row, even
though it would be expected of the second table in the example.
