yes: sometimes = several times

To reproduce you have to follow the steps exactly. If you do so, image #2 will
be rendered on top of image #1!

Can you please try this finer steps to reproduce?

1.) new document
2.) add text to the first paragraph ("aaa aaa aa aaaa aaaaa aaa" ...)
3.) add a empty paragraf 
4.) add another paragraph with text ("aaa aaa aa aaaa aaaaa aaa" ...)
5.) add an image to the empty paragraph, add caption (adds image-number-field)

6.) mark & copy starting from the middle of the first paragraph to the middle
of the last paragraph (image included)
7.) Set Cursor to the end of the document after the last character
8.) paste clipboard several times at the end of the document

9.) mark & copy starting from the middle of the third-last paragraph (=text) to
the middle of the last paragraph (=text) (last image included)

10.) paste it several times at the end of the document
12.) Check the field numbers
