Attached government RTF document does not display correctly:

registration statement

Page 2 extends over to page 3.
Table on page 3 extends past right page boundary.
Check boxes all checked; checkboxes should be unchecked.
Lots of blank pages at the end of the document making it a 27 page document;
should be 4 pages in total.
