The likely place for a chapter index is at the beginning of that chapter. The
Heading of the chapter then is repeated in the first line of the index.
I would like to avoid that repetition and suggest not to include the chapter
heading in a chapter index or to make inclusion optional.
If excluded the user still has the option to write an appropriate text in the
index title.
